2026 Holiday Program
Addison Township Human Services is now accepting applications for our 2026 Holiday Program. The program provides holiday assistance to eligible Addison Township families with children ages 0–15.
Registration
October 7 – November 20, 2026
Families may apply online or visit Addison Township to complete an application in person.
Online applicants must bring all required documents to Addison Township within 2 weeks of submitting their application for their application to be considered complete.
Required Documents
Applicants must provide:
- Valid photo ID
- Proof of Addison Township residency (utility bill or piece of mail dated within the last 30 days)
- Birth certificates for children ages 0–15 or DHS Medical Card showing birthdates
- Proof of income at or below 200% of the Federal Poverty Level, OR SNAP/Medicaid Eligibility Letter, OR Addison Township Food Pantry use
- Additional documentation may be requested to determine eligibility
